The Jewish Home Care Agency- Medical Home Care 

Our newest community-based program is now operational and accepting new medical home care clients in the community.  The Jewish Home Care Agency is awaiting the final level of survey and is currently able to accept new patients.   In coordination with the client’s physician, the team will develop a plan of care that promotes restoration of health and independence during an illness, or following an injury or surgical procedure. 

Services include:

• Home Health Aide Services
• Caregiver Education & Support
• Medical Social Work
• Physical therapy
• Occupational therapy
• Speech therapy
• Nutritional Counseling
• Case Management Services
• In-Home Safety Assessment
